About the role
The Patient Care Navigator functions to provide access to quality care for patients diagnosed with cancer. In conjunction with the registered nurse, the Patient Care Navigator builds trusting relationships with patients through guidance, engagement, coordination of care, resource allocation, and assists with questions for the healthcare team.
Responsibilities
- Builds trusting relationships with patients through guidance, engagement, coordination of care, resource allocation, and assists with questions for the healthcare team.
- Serves as a point of contact to patients assisting with support throughout the multidisciplinary nature of cancer treatment.
- Works in alignment with the nurse navigation department, assisting the team in areas that may include illness perception, care coordination, and appointment assistance.
- Demonstrates a model of service, team collaborator, and interacts with other co-workers and patient's caregivers.
- Stays current with organizational resources and available community resources for the patient population the organization serves.
Requirements
- Education: High School Diploma or Equivalent Required, Bachelor's Degree Preferred
- Completion of Patient Care Navigator education program preferably from George Washington University or American Cancer Society Preferred
- Experience: 2 years experience in a healthcare setting or equivalent preferably in oncology Required
- General Experience and Knowledge: Basic clinical concepts, EPIC systems, complex patient issues and social determinants of health, patient outreach methods and strategies, medical terminology, patient navigator/community health worker experience
Qualifications
- Preferred: Completion of Patient Care Navigator education program from George Washington University or American Cancer Society
- Preferred: Experience in oncology
- Preferred: Experience with medical terminology
- Preferred: Experience working as a patient navigator/community health worker
