Patient Care Manager, Hospice RN

Position Overview

The Director of Clinical Services oversees day-to-day clinical operations for our hospice program, ensuring the delivery of high-quality, compliant, patient-centered care. This role provides leadership and support to hospice clinicians while partnering closely with operations and executive leadership on strategic growth, quality initiatives, compliance, staffing, and patient care outcomes.

Key Responsibilities

• Lead, coach, and support a team of hospice RNs and CNAs

• Serve as a clinical resource for symptom management, complex care planning, and end-of-life care decisions

• Foster a positive team culture focused on compassion, accountability, collaboration, and excellence

• Participate in an on-call leadership rotation (approximately 5 days per month)

• Maintain a small patient caseload to stay connected to clinical care

Qualifications

• Required:

  • Active Minnesota Registered Nurse (RN) license in good standing

  • Minimum 3 years of hospice experience

  • Minimum 2 years of hospice leadership, supervisory, or management experience

  • Strong knowledge of hospice regulations, interdisciplinary care, and end-of-life best practices

  • Excellent leadership, communication, organizational, and problem-solving skills

  • Current CPR certification

  • Valid driver's license, reliable transportation, and auto insurance

• Preferred:

  • QAPI, infection control, and regulatory surveys experience

  • EMR/documentation system experience
