Hospice RN Patient Care Manager

Role: The Patient Care Manager - Hospice oversees patient care activities, ensuring safe, efficient, and compliant delivery of services in accordance with nursing, regulatory, medical, and payer standards. Key Responsibilities Ensure compliance with hospice policies, procedures, and regulatory requirementsPromote and supervise quality of care and safety for patients and staffInvestigate complaints and resolve problems effectivelyConduct staff in-services on health, safety, regulatory issues, and policiesProvide care in diverse home situations, respecting all clients regardless of background Qualifications & Skills Unrestricted RN license in the practicing stateAt least 1 year of clinical experience; 4+ years preferredValid driver’s license, vehicle insurance, and reliable transportationStrong communication, organizational, and writing skillsAbility to work independently; CPR certification preferred or achievable within 90 days The role offers competitive compensation, benefits, and growth opportunities, emphasizing a culture of caring, connecting, and growth.
