Patient Care Manager - Home Health North Augusta

Responsibilities

• Supervise and coordinate clinical services for home health patients.
• Ensure the continuity of high quality care as per the physician’s plan of care.
• Comply with all applicable state and federal laws and regulations.

Qualifications

• Graduate from a recognized, accredited school of nursing.
• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure upon hire.
• CPR - Cardiac Pulmonary Resuscitation upon hire.
• DL NUMBER - Driver License, valid and in state current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation upon hire.
