Patient Care Manager - Home Health Administration

Responsibilities

• Captures the overall supervision and coordination of clinical services.
• Serves as a liaison between the interdisciplinary team and the patient’s healthcare providers.
• Safeguards the continuity of high-quality care for home health patients within the assigned area.

Qualifications

• Graduate from a recognized, accredited school of nursing.
• Hold a current RN - Registered Nurse license from Georgia State.
• Obtain and maintain multistate licensure if required.
• Pass CPR certification.
• Hold a valid and current Driver’s License, vehicle insurance, and access to a reliable vehicle or public transportation.
