Responsibilities
- Provides clinical services within the scope of practice, as defined by state laws governing nursing, in accordance with the plan of care and in coordination with other healthcare team members.
- Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and schedules start of care visits.
- Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensures patient needs are continually assessed and care rendered is individualized, appropriate, reasonable, meets home health eligibility criteria, and adheres to physician orders.
- Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations as appropriate.
- Functions effectively in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Qualifications
- Current unrestricted RN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- State Specific Requirement: RN licensure must have no restrictions (AK, AL, AR, AZ, CO, CT, DE, FL, GA, ID, IL, IN, KY, MA, MI, MD, MN, MO, MS, NC, NH, NJ, NM, NV, NY, OH, OK, OR, PA, RI, SC, TN, TX, VA, WA, WI, WV).
Preferred Qualifications
- Current CPR certification or ability to complete within 90 days of hire.
- Home care experience.
Pay
The salary for this role ranges from $89,680 to $134,520 annually based on full-time employment. Pay is based on several factors including local labor markets, education, work experience, certifications, etc.
