Patient Care Manager
Primary Responsibilities
- Coordinates referrals, authorizations, start-of-care (SOC) visits, and clinician assignments to ensure timely, appropriate patient evaluations
- Manages ongoing patient care coordination, including benefits verification, medical necessity, insurance approvals, and payer change documentation
- Oversees clinical documentation quality and timeliness, including OASIS assessments, visit notes, orders, care plans, and productivity standards
- Serves as a central communication hub with physicians, clinicians, patients, families, referral sources, and interdisciplinary partners to ensure continuity of care
- Maintains oversight of patient status, labs, diagnostic results, on-call events, and significant condition changes, ensuring physician notification and plan-of-care updates
- Facilitates case conferences, admissions, transfers, discharges, and interdisciplinary collaboration to ensure appropriate utilization of services and resources
- Provides clinical leadership through supervision, orientation, quality oversight, risk reporting, and support of staff performance and regulatory compliance
Required Qualifications
- Current unrestricted RN licensure in state of practice
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
- State-specific requirements for RN licensure may apply
Preferred Qualifications
- Current CPR certification or ability to complete within 90 days of hire
- Home care experience
Pay and Benefits
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.
In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
The salary for this role will range from $85,196 to $127,794 annually based on full-time employment.
