Patient Care Manager

About the role

The Patient Care Manager at Caring Hospice is responsible for coordinating all clinical services provided to hospice patients, including those in their homes and nursing facilities. This role requires oversight of the day-to-day operations to ensure they align with the organization's mission, philosophy, and goals, while also adhering to relevant laws, rules, and regulations.

Responsibilities

• Coordinate and direct all clinical services provided to hospice patients
• Oversee the day-to-day clinical operations of hospice
• Ensure services comply with organizational mission, philosophy, and goals
• Adhere to all applicable laws, rules, and regulations
• Report to: Program Director

Requirements

• Licensed as a Registered Nurse in PA
• A Bachelor’s or higher degree in nursing or a related health field preferred
• Two to four years of clinical experience in a home health agency, home care setting, or health facility
• Two to four years of experience in a supervisory or administrative position in a hospice organization

Qualifications

• Strong knowledge of hospice regulations
