Patient Care Coordinator - Vascular Surgery

Join a dedicated Vascular Surgery team where you’ll play a key role in guiding patients through every step of their care journey. As a Patient Care Coordinator, you'll be the connection between patients, providers, and clinical teams—helping ensure seamless scheduling, care coordination, and an exceptional patient experience in a fast-paced specialty clinic environment.
Responsibilities
- Coordinate patient care activities, ensuring timely and accurate scheduling of appointments and procedures.
- Communicate effectively with patients, physicians, and other healthcare professionals to facilitate smooth transitions in care.
- Act as a patient advocate to ensure patients have access to necessary health care services.
- Manage patient information securely and maintain confidentiality as required by HIPAA and other regulations.
- Collaborate with interdisciplinary teams to develop and implement care plans that meet patient needs.
Requirements
- 1 year Experience in medical/healthcare setting or two years in a customer service role.
- Experience providing a high level of customer service in a fast-paced environment, including handling confidential data with discretion.
Qualifications
- High school diploma or equivalent.
- Completion of a medical assistant program preferred but not required.
- Basic computer skills, including proficiency with electronic medical records (EMR).
Skills
- Excellent interpersonal and communication skills.
- Strong organizational and time management abilities.
- Ability to handle sensitive and confidential information.
- Proficiency in Microsoft Office Suite.

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
