Responsibilities

• Oversee the coordination of patient care while serving as a primary contact for patients and their families.
• Assist with scheduling, transportation, and connecting patients to community resources and support programs.
• Track patient progress, while identifying and resolving care coordination gaps to enhance patient outcomes.
• Participate in ongoing training related to care coordination and quality.

Requirements

• Bachelor's Degree required.
• 2-4 years of related experience required.
• Strong understanding of healthcare systems, insurance, and community resources.
• Strong organizational and communication skills.
• Ability to collaborate effectively across teams.
• Commitment to patient advocacy and compliance.

Schedule

• Full-time (Monday-Friday, 8:00 AM - 5:00 PM).
• Schedules are established early and rarely change, so employees enjoy a steady workweek rhythm.
• No routine nights or weekends for most positions (some clinical roles may have limited weekend coverage based on clinic needs).
