About the role

As a Patient Care Coordinator, you will play a vital role in ensuring comprehensive care coordination for HIV/AIDS clients, combining traditional case management with enhanced patient coordination services. The Patient Care Coordinator-Infectious Disease will work closely with patients, their families, healthcare providers, and other stakeholders to facilitate communication, manage care plans, and provide necessary resources, referrals, behavioral health support, coordinated care, and insurance assistance.

Responsibilities

  • Oversee the coordination of patient care while serving as a primary contact for patients and their families.
  • Assist with scheduling, transportation, and connecting patients to community resources and support programs.
  • Track patient progress, identify and resolve care coordination gaps to enhance patient outcomes.
  • Participate in ongoing training related to care coordination and quality improvement.

Requirements

  • Bachelor’s Degree required.
  • 2-4 years of related experience required.
  • Strong understanding of healthcare systems, insurance, and community resources.

Schedule

Full-time, Monday-Friday, 8:00 AM - 5:00 PM.
