About the role
Patient Care Coordinator – Home Healthcare At Peace Healthcare Agency is seeking an experienced Patient Care Coordinator to join our growing home healthcare team. Spanish-speaking is preferred. We are looking for a highly organized, compassionate, and dependable professional with experience in home healthcare, patient care coordination, caregiver scheduling, intake, or healthcare administration.
Responsibilities
- Coordinate patient care and caregiver schedules
- Communicate with patients, families, caregivers, and healthcare professionals
- Manage schedule changes, call-offs, and staffing needs
- Follow up on patient needs, concerns, and satisfaction
- Maintain accurate documentation and schedules
- Assist with intake and service coordination
- Protect patient confidentiality and follow HIPAA requirements
Qualifications
- Spanish-speaking is a plus
- Previous home healthcare/home care experience REQUIRED
- Experience with patient care coordination, scheduling, intake, or staffing
Skills
- Excellent communication and organizational skills
- Ability to multitask in a fast-paced environment
- Proficient with computers and healthcare/scheduling software

Pay
$18–$23 per hour
