Summit Home Care is seeking experienced Patient Care Coordinators who speak fluent English and Spanish for our Brooklyn office.

About the role

Patient Care Coordinators match patients with the right home health aides, provide basic case management services, and maintain communication with patients, office personnel, paraprofessional staff, and contract agencies.

Responsibilities

• Match patients with appropriate home health aides
• Provide basic case management services
• Maintain communication with patients, office personnel, paraprofessional staff, and contract agencies
• Maintain accurate schedules, notes, and records

Requirements

• Minimum of one year of experience in home care coordination
• Must speak fluent English and Spanish
• HHA Exchange experience is required
• Excellent telephone and communication skills
• Excellent organizational skills
• Good computer skills
• Good interpersonal skills
