About the Role
Kindred Hospital Las Vegas - Sahara is a 52-bed long-term acute care hospital offering extended recovery care. We provide 24-hour clinical care seven days a week and feature an Outpatient Wound Care Clinic. This role involves conducting patient assessments for potential admission, maintaining relationships with referral sources, and coordinating clinical and benefits information for admissions into ScionHealth Specialty Hospitals.
Responsibilities
- Conduct patient assessments in accordance with company standards.
- Educate patients and their families on the benefits of Kindred Hospital and assist in hospital tours.
- Facilitate timely admissions by:
- Identifying necessary medical record information from referring facilities.
- Completing the Pre-Admission Clinical Evaluation (PACE) form to assess admission appropriateness.
- Coordinating physician-to-physician communication.
- Identifying patient needs for equipment, supplies, and consult services.
- Collaborate with management, participate in marketing meetings, and serve on internal committees as assigned.
- Maintain up-to-date knowledge of areas of responsibility through continuing education.
- Adhere to company standards, including the Code of Business Conduct, policies, procedures, and regulatory requirements.
- Ensure confidentiality and compliance with all regulatory requirements for patient information.
- Perform other related duties as assigned.
Requirements
- Ability to collect, organize, evaluate, and communicate clinical information effectively both verbally and in writing.
- Strong customer service skills and commitment to service excellence.
- Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint).
- Excellent presentation and communication skills.
- Clinical background with the ability to conduct patient assessments.
- Ability to work under stress and respond quickly in crisis situations.
- Knowledge of healthcare marketing and plan development.
- Ability to work cooperatively as part of a team.
- Must read, write, and speak fluent English.
- Good and regular attendance is required.
- Travel requirements vary by region.
Qualifications
- Associate or Bachelor’s degree; healthcare-related field preferred.
- Healthcare professional licensure preferred (e.g., Registered Nurse, Licensed Practical Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Social Worker).
- Two (2) years of clinical patient care/treatment experience in a healthcare field preferred.
- Demonstrated understanding of Medicare, Medicaid, and other payers related to LTACH benefits.
Pay
$30.00 - $38.05 per hour.
