Role Summary

Conducts patient assessments in accordance with company standards for appropriate patients for potential admission into the system. Maintains positive relationships with referral sources, sales teams, admission, managed care, clinical and operational teams. Coordinates transmission of clinical and benefits information of patients for possible admission into our ScionHealth Specialty Hospitals.

Essential Functions

  • Conduct patient assessment in accordance with company standards.
  • Work closely with patients and their family members to educate them on the benefits of Kindred Hospital.
  • Affixes the timely admission of patients from the referral sources:
    • Identifies medical record information needed from referring facility;
    • Completes the Pre-Admission Clinical Evaluation (PACE) form to assist in evaluating appropriateness of admission;
    • Facilitates and coordinates physician-to-physician communication as necessary;
    • Identifies specific patient needs for equipment, supplies and consult services
  • Maintains a state-of-the-art knowledge of areas of responsibility and develops and follows a program of continuing education.
  • Meets with management staff, participates in marketing meetings and serves on various internal committees as assigned.
  • Operates with a sense of urgency to produce quick results.
  • Maintains all patient information in a manner that ensures confidentiality and meets all regulatory requirements.

Knowledge/Skills/Abilities/Expectations

  • Ability to collect, organize, evaluate and communicate pertinent clinical information with effective verbal and written skills.
  • Strong customer service skills and commitment to service excellence.
  • Computer skills that include Microsoft Office, Word, Excel and Outlook and PowerPoint.
  • Excellent presentation and communication skills.
  • Clinical Background with ability to conduct Patient Assessments.
  • Ability to work under stress and to respond quickly in crisis situations.
  • Knowledge of healthcare marketing and plan development.
  • Ability to work cooperatively as a member of a team.

Qualifications

  • Associate or Bachelor’s degree
  • Degree in healthcare related field preferred
  • Healthcare professional licensure preferred.
  • Registered Nurse, Licensed Practical Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Social Worker license
  • Two (2) years clinical patient care/treatment experience in healthcare field preferred
  • Demonstrated understanding of Medicare, Medicaid, and other payers as related to LTACH benefits
