About the Role
The Patient Advocate (PaD) facilitates and ensures continuity in patient care, serving as a conduit between the health center's services and the community. The PaD strategically engages employees and their dependents before, during, and after visits to provide personalized, informative, and compassionate care. This role works closely with the Vice President of Service Delivery, Account Manager, Medical Assistant, Provider, and Client HR Team.
Responsibilities
- Coordination of Care
- Coordinate care with external specialists, labs, medical equipment providers, counselors, and health classes under the direction of the Provider and Sr. Patient Advocate.
- Ensure electronic medical records are provided to Proactive MD’s Provider following any specialist visit.
- Obtain prior authorizations required by insurance companies for tests or procedures.
- Assist patients in navigating the healthcare system.
- Coordinate care during patient admissions and discharges from the emergency room or hospital, ensuring Proactive MD’s Provider is included in the aftercare process.
- Visit hospitalized patients to ensure care coordination is directed by the Provider.
- Encourage and coordinate follow-up visits at the health and wellness center.
- Analyze data and develop outreach campaigns and educational resources in coordination with the on-site Provider.
- Represent the health and wellness center at internal and external meetings and events.
- Patient Education
- Provide information to patients about available community services that could enhance their care.
- Coordinate patient care and treatment plans; provide educational materials and consultation.
- Follow up on patient well-being.
- Reporting & Analytics
- Analyze data to identify population risk management trends and develop outreach campaigns and programs in collaboration with the Provider.
- Evaluate historical claims data and collaborate with physicians on clinical priorities.
- Develop programs, outreach campaigns, and educational resources to encourage health and wellness center utilization.
- Assist with client relations and generate reports for the Account Manager.
Requirements
- 2+ years of experience working in a healthcare setting, Medical or Healthcare Social Work in a similar setting, or a Case Management role.
- Bachelor of Social Work Degree.
- Knowledge of the concepts and principles of case management.
- Ability to communicate effectively and maintain working relationships with people from diverse backgrounds.
- Knowledge of the healthcare industry and available patient resources.
- Understanding of payors and healthcare billing processes.
- Ability to prioritize needs and plan work accordingly.
- Knowledge of HIPAA confidentiality requirements.
