Patient Advocate
About the role
The Patient Advocate & Liaison (PAL) is the face of Matter Health within the communities we serve. This field-based role is responsible for building trusted relationships with older adults living in affordable housing communities and helping them stay engaged in their healthcare journey. Working alongside a Nurse Practitioner (NP), you'll serve as the primary relationship-builder for your assigned communities—educating patients about Matter Health, supporting enrollment, coordinating follow-up, identifying barriers to care, and helping patients stay connected to the services they need. This role is ideal for someone who enjoys working directly with people, thrives in community settings, and is passionate about improving healthcare access for underserved populations.
Responsibilities
- Build trusted, lasting relationships with older adults living in assigned communities
- Serve as the primary community representative for Matter Health
- Educate prospective patients about Matter Health's care model and available services
- Support patient outreach, enrollment, scheduling, and ongoing engagement
- Conduct in-person outreach, phone calls, and community events to connect with patients
- Partner closely with Advanced Practice Providers (APPs), Social Workers, Care Navigators, and Operations teams
- Identify barriers to care and connect patients with appropriate community resources
- Assist patients throughout their healthcare journey—from initial introduction through ongoing engagement
- Maintain accurate documentation in Salesforce and Athena
- Help improve patient retention and overall healthcare access within your assigned communities
Requirements
- 1–2 years of experience in community outreach, patient advocacy, healthcare, social services, customer service, hospitality, account management, sales, or other relationship-focused roles
- Passion for serving older adults and underserved communities
- Outstanding communication and relationship-building skills
- Comfortable working independently in a field-based environment
- Strong organization and time management skills
- Ability to collaborate with clinical and operational teams
- Valid driver's license and reliable transportation required
Qualifications
- Experience working with Medicare populations, community organizations, Salesforce, Athena, or healthcare outreach is a plus
- Bilingual Spanish is a plus

Pay
Compensation Range: $61K - $74K
