About the role

The Patient Access Coordinator serves as a member of a centralized triage and referral line under Project LAUNCH. Duties include centralized scheduling, managing referrals, and assisting clients and families with resources needed to support their service needs and overall mental health. This role prioritizes a trauma-informed and culturally competent approach to understanding and working with individuals in need of mental health support. Information is gathered via phone triage assessments to link individuals or families with needed services, including support not provided through the LECOM Health System. All duties are completed with strict confidentiality and in compliance with HIPAA and other agency, state, or federal rules. This position reports to the Centralized Access Supervisor.

Responsibilities

  • Assess the needs of children, adults, and families while adhering to HIPAA and all confidentiality requirements.
  • Serve on behalf of a centralized intake and referral line, processing referrals initiated by individuals, families, internal LECOM providers, or external stakeholders.
  • Gather information on specific needs and preferences for services and supports, ensuring referral sources are aware of any barriers to timely linkage to care.
  • Complete client registration, authorization, and eligibility determinations.
  • Provide families with information regarding available LECOM Behavioral Health or Physical Health services, reviewing steps to access care.
  • Schedule appointments to assist individuals and families in completing necessary intake assessments or evaluations.
  • Uphold trauma-informed perspectives and practices during all activities and interactions.
  • Actively support the mission and values of the agency, contributing positively to workplace culture.
  • Participate in required SAMHSA meetings, audits, trainings, or other events as assigned.
  • Engage in committees, grants, special projects, and other duties as assigned.
  • Maintain professional relationships with community service providers, stakeholders, and referral sources.

Requirements

  • High school diploma or GED.
  • Associate’s degree or higher in medical billing or a human services field is preferred.
  • Two years of experience working in social serving systems is required.
  • Additional experience in customer service, patient registration, or medical billing is preferred.
  • Multilingual or cultural diversity experience is a plus.

Schedule

This is a part-time position (25–30 hours per week), scheduled Monday through Friday, with flexibility in work hours to accommodate program and business needs.
