About the Role
The incumbent will supervise non-clinical discharge coordinators and support staff, assisting with the management of personnel, fiscal, and material resources. This includes performance management, overseeing day-to-day tasks of discharge coordinators, and staff development. The role is critical in maximizing care and ensuring safe patient discharges from the UCSF General Medicine Service, a direct care service without learners.
The incumbent will collaborate closely with the Division Chief, Medical Director, Assistant Medical Director, Operations Manager, and Division Manager. They will act as a liaison between discharge coordinators and outpatient clinics regarding patient appointments and other patient-based activities.
Key responsibilities include addressing administrative clinical issues, optimizing post-discharge follow-up care, obtaining outside records, enhancing communication between inpatient and outpatient providers, and providing additional patient support. The role also involves following up on critical lab results (e.g., microbiology, pathology) to ensure Primary Care Providers receive them and patients are scheduled for appropriate follow-up care.
Additionally, the incumbent will follow up with patients post-discharge to address hospitalization-related concerns, such as triaging calls for the post-discharge phone line and answering patient questions.
Responsibilities
- Supervise non-clinical discharge coordinators and support staff, including performance management and day-to-day oversight.
- Manage personnel, fiscal, and material resources for the discharge coordination team.
- Liaise with outpatient clinics to coordinate patient appointments and other activities prior to discharge.
- Address administrative clinical issues to augment patient care on the service.
- Determine optimal times for post-discharge follow-up care and obtain relevant outside records.
- Enhance communication between inpatient and outpatient providers.
- Follow up on critical lab results (e.g., microbiology, pathology) to ensure Primary Care Providers receive them and patients are scheduled for follow-up care.
- Provide additional support to patients as needed.
- Follow up with patients post-discharge to address hospitalization-related concerns, including triaging calls for the post-discharge phone line and answering patient questions.
- Perform other duties as assigned.
Requirements
- Bachelor's degree in a health-related area and/or equivalent experience/training.
- Thorough knowledge of medical center services, processes, and procedures.
- Thorough knowledge of University Medical Center policies.
- Thorough knowledge of relevant federal, state, and local regulations to identify potential patient risk.
- Knowledge of medical terminology.
- Advanced knowledge of data collection, compilation, and analytical techniques.

Department Overview
The Department of Medicine (DOM) is the largest department in the UCSF School of Medicine, with a mission encompassing research, teaching, patient care, and public service. The department has a decentralized structure across four sites (Parnassus, SFGH, VAMC, Mt. Zion) and includes approximately 38 divisions.
The Division of Hospital Medicine (DHM) is the largest division in the DOM, recognized as a national leader in clinical care, education, research, and quality improvement. Clinically, the division manages 14 services and provides care for nearly half of the adult hospital patients daily. The Medicine Service, one of its services, is the largest inpatient service at UCSF Medical Center, with close to 11,000 annual patient discharges. The division comprises 180 faculty, 12 MSP Physicians, 9 fellows, and 34 staff, and engages a cadre of 100 external physicians, fellows, and residents for moonlighting services.
