PASRR Clinical Evaluator (LPCC, LMHC, LMFT, LCSW, NP, or PMHNP)

About the Role

Acentra Health is seeking a PASRR Clinical Evaluator to conduct Preadmission Screening and Resident Review (PASRR) Level II assessments, make level-of-care recommendations, and ensure quality of care meets industry standards and government regulations. This full-time, hybrid position involves telehealth and in-person assessments at nursing facilities and hospitals in the Yakima, Washington area.

Responsibilities

• Complete accurate PASRR Level II assessments by reviewing records, gathering collateral information, and evaluating physical, behavioral, and psychological functioning.
• Develop evidence-based level-of-care recommendations and determine appropriate next steps for individuals requiring long-term care.
• Review and verify medical records, providing clinical recommendations in compliance with regulatory requirements, including psychiatric input, physician notes, and timely assessments.
• Interact with healthcare professionals, patients, families, and internal partners to support the assessment process.
• Leverage technology, web-based applications, and proprietary software to manage and track assessment activities.
• Ensure compliance with workflows, contractual obligations, and program requirements while completing assessments within scope of practice.
• Determine clinical diagnoses, accounting for psychological, behavioral, and physical functioning.
• Adhere to corporate policies, including HIPAA Privacy and Security Rules.

Requirements

• Active, unrestricted Washington state licensure in a healthcare or related field (e.g., LPCC, LMHC, LMFT, LCSW, NP, or PMHNP).
• Graduation from an accredited college or university with a degree in a relevant field.
• 3+ years of experience assessing or diagnosing individuals with mental or psychiatric disorders, including complex cases involving substance use, medical conditions, cognitive impairment, dementia, and geriatric populations.
• 1+ years of experience working with individuals with intellectual and developmental disabilities, serious mental illness, or related conditions.

Preferred Qualifications

• Strong clinical judgment, evidence-based assessment, and critical-thinking skills.
• Proficiency in medical record abstraction and review, including medical terminology and disease processes.
• Knowledge of Federal Regulation 42 CFR, Chapter 483 and psychiatric disorders as defined by the current Diagnostic and Statistical Manual.
• Understanding of standards of practice for level-of-care determinations, mental health, intellectual/developmental disabilities, geriatrics, and long-term care.
• Excellent communication skills (spoken, written, and electronic) with individuals and groups.
• Strong organizational skills, attention to detail, and ability to manage priorities under deadlines.
• Proficiency in Microsoft Office Suite (Excel, Word, PowerPoint, Outlook, Teams) and web-based tools/platforms.

Pay

USD $74,480.00 – USD $93,600.00 per year. Placement within this range depends on years of applicable experience and skill level.
