Part-Time Wound Care Nurse LPN/RN

We are currently seeking a full‑time wound care RN/LPN to join the team at Douglasville Center Nursing and Rehabilitation. The wound care nurse participates in assessing and evaluating the clinical needs of residents with wounds, and works with the resident and the rest of the care team in planning and implementing care that addresses those needs. This work supports the mission of Woodstock Nursing and Rehabilitation Center.

Responsibilities

• Conduct comprehensive wound consultations, including detailed wound assessments and recommendations for interventions and care planning.
• Perform wound assessments for residents and complete all associated documentation, tracking, and care‑planning activities.
• Provide ongoing monitoring of all assigned residents with wounds.
• Assess resident and caregiver knowledge of wound prevention and treatment.
• Provide resident and caregiver education on wound prevention and treatment as needed.
