About the role
We're looking for a Clinical Navigator (Licensed Practical Nurse) to support Medicare patients in our care programs. This role involves proactive outreach, coaching, and care coordination.
Responsibilities
- Conduct outbound telephonic patient outreach
- Provide one-on-one coaching and support to patients managing chronic conditions
- Help patients execute personalized care plans and achieve their goals
- Conduct comprehensive assessments of patients' health status and lifestyle behaviors
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies
- Maintain accurate clinical records and document patient interactions
- Collaborate with clinicians and patient support teams to ensure smooth workflow and patient engagement
Requirements
- An active compact multi-state LPN/LVN license in the state where you reside
- At least 5 years of clinical experience as a Licensed Practical Nurse
- Remote work readiness (stable high-speed WiFi)
- An active CA state LVN license
- Fluency in Spanish is preferred
- Experience providing patient education and support to individuals managing chronic conditions
- Experience using AI-assisted clinical tools

Pay
The anticipated OTE range for this role is $26 - $31 per hour.
Schedule
This is a part-time, remote position based in the United States. Candidates must be available to work at least three days per week during standard business hours of either 8:00 AM to 5:00 PM or 9:00 AM to 6:00 PM Pacific Time. Availability to work Saturdays (either 8:00 AM to 5:00 PM or 9:00 AM to 6:00 PM Pacific Time) is required.
