Part-Time Weekend Physical Therapist

Responsibilities

• Evaluate patients' functional status, mobility, endurance, pain, and medical conditions
• Develop and implement personalized treatment plans in collaboration with healthcare team and physicians
• Perform interventions to improve patient well-being, restore function, and relieve pain
• Monitor and report patient progress and response to treatments
• Educate patients, families, and team members about the care plan, limitations, and progress
• Complete patient evaluations, progress notes, and develop plans within state guidelines
• Communicate outcomes, goals, and care frequency to relevant team members

Qualifications And Skills

• Current Physical Therapy license in the practice state
• CPR certification
• Valid driver's license and access to transportation
• Ability to work effectively in diverse home environments and with varied patient needs
