About the role

Catholic Charities of Kansas City-St. Joseph seeks a part-time supportive services coordinator to provide services to residents of Wilshire Hills III, a senior housing complex.

Responsibilities

  • Complete a comprehensive assessment with residents to determine their individual needs and design goals.
  • Develop individual case plans with new residents within two weeks of intake.
  • Design and implement tasks and activities to foster a sense of community among residents.
  • Conduct life skill and budgeting sessions with residents, either individually or in groups.
  • Coordinate case management efforts with all staff and external services.
  • Operate as the primary referral agency for residents, coordinating both on-site services and referrals to social services.
  • Develop and oversee a peer-to-peer mentoring program for residents.
  • Maintain detailed files on each resident, including eligibility documents, strengths assessments, personal goal plans, and progress notes.
  • Collect and maintain comprehensive statistical data on residents' needs and progress.
  • Track financial assistance provided to residents and monitor goal progress.
  • Maintain accurate records and prepare case records for proper storage after residents exit the program.
  • Submit monthly expenditure reports and vouchers.
  • Build and maintain partnerships with community organizations to support residents' needs.
  • Stay informed about current trends and resources available to residents.
  • Attend agency and community meetings as requested.
  • Participate in in-service training and workshops as needed.
  • Complete other reports as requested.

Requirements

  • Bachelor’s degree in Social Work, Gerontology, Psychology, or Counseling (preferred); equivalent work experience may be considered.
  • Knowledge of and training in family, elderly, and disability services.
  • Experience in referral processes.
  • Understanding of local services for the specific resident profile and people with disabilities.
  • Two to three years of social service delivery experience, preferably in direct work with families, elderly people, or people with disabilities.
