Part Time RN Home Visits

About the Role

This position involves providing and coordinating home health nursing care, ensuring compliance with laws, regulations, and agency policies. The RN will assess patients, develop and revise individualized care plans, and collaborate with healthcare professionals, patients, families, and community resources to ensure continuity of care.

Responsibilities

• Deliver high-quality clinical nursing services within scope of practice and infection control standards
• Coordinate care from admission to discharge with the care team
• Conduct clinical assessments in line with federal, state, and payer requirements
• Verify patient eligibility and medical necessity for services
• Develop and update personalized care plans in collaboration with community providers
• Adjust care plans as needed with physician approval

Requirements

• Current, unrestricted RN licensure in the practice state
• Valid driver’s license, vehicle insurance, and reliable transportation or public transit access
• CPR certification
• Ability to adapt to various home environments regardless of client background

Qualifications

Preferred: 1+ years of RN experience, strong communication and organizational skills, and ability to work independently.

Pay

Hourly pay ranges from $39.45 to $59.15, based on experience and location.
