About the role
This outpatient role requires an experienced, independent RN to be an essential part of the interdisciplinary care team.
Responsibilities
- Admitting patients
- Conducting assessments
- Coordinating care
- Managing discharges
- Pronouncements for pediatric patients and their families
Requirements
- Valid Texas RN license (required)
- Over 1 year of RN experience (preferred)
- Experience with Hospice or Home Health (preferred)
- Strong written and verbal communication skills
- Reliable transportation for travel
- Clean background, drug screen, and driving record
