About the role

As a Licensed Field Nurse at Unifi Healthcare, you will provide comprehensive, patient-centered care tailored to each client’s individualized service plan. You will play a critical role in promoting independence, ensuring safety, and enhancing quality of life for our clients and their families.

Responsibilities

Direct Patient Care

  • Provide compassionate personal care including bathing (bed bath, shower, or tub), dressing, grooming, oral care, and hygiene support
  • Assist with mobility including ambulation, wheelchair use, and safe transfers
  • Support toileting needs, including catheter and ostomy care
  • Reposition bedridden clients and maintain clean, comfortable bedding
  • Assist with meal preparation and support nutritional intake as needed

Clinical Responsibilities

  • Monitor and record vital signs (heart rate, blood pressure, respiratory rate, temperature)
  • Document intake and output
  • Administer and manage medications as prescribed
  • Assess client nursing needs on an ongoing basis
  • Provide nursing services in accordance with the established care plan
  • Promptly report changes in condition, concerns, or progress to the Case Manager and physician
  • Complete accurate and timely clinical documentation

Care Plan Collaboration

  • Participate in the development and implementation of individualized service plans
  • Observe and report changes in mood, behavior, or emotional status
  • Communicate effectively with interdisciplinary team members

Supportive Services

  • Perform light housekeeping to maintain a safe, healthy environment (laundry, vacuuming, dusting, kitchen tasks)
  • Organize client living spaces to promote safety and comfort
  • Assist with errands and transportation, including medication pickup and grocery shopping

Professional Standards

  • Practice strictly within legal scope of licensure
  • Maintain professional appearance and boundaries
  • Promote teamwork, unity, and a culture of respect
  • Adapt to additional responsibilities as assigned by leadership

Requirements

  • Current, valid RN or LPN/LVN license
  • Reliable personal vehicle with valid insurance
  • Clear criminal background check and negative pre-employment drug screening
  • Strong verbal and written English communication skills
  • Must be able to lift at least 25 pounds

Working Environment

  • Daily travel to client homes and occasional visits to the agency office
  • Flexible scheduling, including potential weekends and holidays (with compensation provided)
  • Work environment varies based on individual client needs
