About the role

This role provides high-quality, comprehensive nursing care, managing multidisciplinary healthcare services for a designated caseload. It involves assessing patients’ physical, social, and psychological needs to develop and implement personalized care plans, ensuring adherence to policies, standards, and reimbursement protocols. The nurse acts as a care coordinator, facilitating referrals, ordering supplies, documenting all services, and maintaining communication with team members and management. Responsibilities include supervising care delivery, participating in staff orientation and mentoring, transporting medical equipment, and traveling to patients’ homes or facilities with varying environments. The role requires maintaining productivity, engaging in ongoing professional development, and performing special projects as assigned.

Responsibilities

  • Assess patients’ physical, social, and psychological needs and develop personalized care plans
  • Ensure adherence to policies, standards, and reimbursement protocols
  • Coordinate care, facilitate referrals, order supplies, and document all services
  • Maintain communication with team members and management

  • Perform special projects as assigned
  • For specialized programs (CHHA, Hospice, or Nurse-Family Partnership), coordinate care with multidisciplinary teams, supervise licensed staff, manage discharges, and support patient activities of daily living
  • NFP nurses focus on health education, social support, and community linkages for pregnant and postpartum mothers

Qualifications

  • Current registered nurse license in the state
  • Valid driver’s license or NYS Non-Driver ID (may be required)
  • Bachelor’s Degree in Nursing (NFP-specific)
  • Minimum one year of clinical experience in settings such as Med/Surg, ICU, Home Care, or Maternal/Child Health
  • Basic computer skills required
  • Additional certifications (e.g., Wound Care, Hospice & Palliative Care) preferred or required within specified timeframes
