About the Role
Provides comprehensive nursing care to meet patient/client needs, coordinating multidisciplinary health services in line with organizational policies, standards, and regulations. Works under moderate supervision and may include specialized responsibilities for Nurse-Family Partnership (NFP), offering health education, social support, and community linkages for pregnant and postpartum first-time mothers.
Responsibilities
- Assess physical, social, psychological, and environmental needs to develop and implement personalized care plans.
- Coordinate care by making referrals, ordering supplies/services, and ensuring delivery; document all services per regulations and policies.
- Participate in staff orientation, mentoring students, and ongoing professional development.
- Deliver nursing care, utilizing medical equipment, and transporting supplies to patient homes or facilities.
- Travel to various patient environments, maintain productivity, and follow established workflows.
- Establish, implement, and revise care plans with input from patients and team members, ensuring safe discharge, and managing patient activities of daily living.
- For specialized roles (CHHA, Hospice, NFP): oversee care plans, supervise staff, update treatment plans, and coordinate discharge processes based on patient needs.
Requirements
- Current RN license in New York State and valid driver’s license or ID; Wound Care Certification preferred for Hospice roles.
- Degree in Nursing; NFP roles require a Bachelor’s Degree in Nursing.
- Minimum one year of relevant clinical experience; maternal/child health experience for NFP; Med/Surg, ICU, or Home Care experience for hospice.
- Basic computer skills required.
