Part-Time/PRN LPN

About the Role

The Licensed Practical Nurse (LPN) plays an integral role in the success of our team, our community, and creating a place our residents are proud to call home. The primary purpose of this position is to provide high-quality, holistic nursing care in accordance with nursing practice standards and health care regulations governing professional nursing. The LPN may lead or direct licensed and non-professional staff in delivering direct resident care and support functions.

Responsibilities

• Nursing Care Functions
  • Conduct daily nursing functions in accordance with current rules, regulations, and guidelines for long-term care facilities/assisted living communities.
  • Ensure nursing personnel comply with written policies and procedures, including the safety program.
  • Admit, transfer, and discharge residents with documentation of care and services provided.
  • Participate in orienting new residents and family members to the facility.
  • Make rounds with health care providers, Hospice staff, and Home Health staff as needed.
  • Utilize nursing personnel qualifications according to resident care plans, service plans, and health care provider orders.
• Communication/Documentation Functions
  • Complete daily clinical and environmental rounds to evaluate resident care and report significant events.
  • Meet and resolve issues with residents and/or family members; report significant issues to the supervisor.
  • Participate in facility surveys (inspections) as requested by management.
  • Responsible for clinical programs as directed (e.g., dementia, wound care, quality assurance, infection control, admissions, and falls).
  • Complete initial assessments, assist in developing interim care plans, and update/review/transcribe care plans based on nursing needs and health care provider orders.
  • Communicate with health care providers regarding treatment plans, orders, progress notes, etc.
  • Review resident medical records for treatments, medication orders, diets, labs, etc., for implementation and follow-up.
  • Requisition and arrange diagnostic and therapeutic services as ordered by health care providers.
  • Communicate department policies and procedures to personnel, residents, and visitors.
  • Provide oral and written reports (24-hour report) on resident condition changes and other shift activities to nursing management.
  • Complete accident/incident reports and submit them to the Director of Nursing.
  • Complete clinically accurate documentation according to policy.
  • Review interdisciplinary team progress notes to ensure care plans are followed; report concerns to the supervisor.
  • Inform nursing personnel of new admissions and resident care needs; ensure rooms are ready for new admissions.
  • Notify health care practitioners and family members of changes in resident condition.
  • Assist in arranging transportation and third-party services for residents.
• Medication Administration Functions
  • Prepare and administer medications as ordered by health care providers.
  • Communicate with the pharmacy to ensure resident medications are available.
  • Maintain an adequate supply of floor stock medications, supplies, and equipment; report needs to the supervisor.
  • Review medication administration records for completeness and accuracy; report concerns to the supervisor.
  • Dispose of drugs and narcotics according to established procedures.
• Personnel Functions
  • Supervise the nursing unit to ensure personnel perform work assignments according to acceptable nursing standards.
  • Work with the Director of Nursing to resolve staff performance issues, including those leading to termination.
  • Report complaints and grievances to management and collaborate on action plans.
• Safety and Sanitation Functions
  • Ensure protective equipment is available for staff.
  • Maintain clean and sanitary nursing service work areas, resident rooms, and treatment areas.
  • Recommend equipment and supply needs to the Director of Nursing; maintain stock levels of medications, medical supplies, and equipment.
• Perform other job-related duties as assigned.

Requirements

• Must meet all applicable state and federal requirements for the position.
• Current, unencumbered, active LPN/LVN license in the applicable state, in good standing throughout employment with no disciplinary action.
• Preferred: One year of experience in nursing within a home care, assisted living, or long-term care facility.

Working Conditions and Physical Demands

Physical Requirements:

• Walk long distances and stand for extended periods.
• Sustain repetitive movements (e.g., CPR compressions, bagging).
• Maintain physical tolerance (stand/walk/work on feet for 8-12 hours with minimal breaks).
• Lift up to fifty pounds at times.
• Push and pull up to fifty pounds at times.

Cognitive Requirements:

• Execute tasks independently.
• Express ideas clearly and effectively.
• General computer literacy.

Environmental Requirements:

• Potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
• Exposure to ambient room temperatures and lighting.

Other Requirements:

• May be required to work nights, weekends, and holidays.

Supervisory Responsibility

This position ensures resident care is provided according to standards of practice and facility policies by CNAs/Caregivers, though CNAs/Caregivers do not directly report to the LPN.
