Role Overview
The Physical Therapist in Home Health evaluates patients' functional status, mobility, pain, and medical conditions to develop and implement personalized treatment plans. They provide interventions to improve or maintain patient well-being, monitor progress, and collaborate with healthcare team members. The role involves conducting patient assessments, documenting visits, and educating patients and caregivers about care plans and progress.
Responsibilities
- Evaluate patients' physical therapy needs and assist in developing care plans.
- Deliver therapy services within legal scope and plan of care.
- Observe and report patient responses and condition changes.
- Treat patients to reduce pain, restore function, and maintain maximum capability.
- Educate patients, families, and healthcare team members about treatment and progress.
- Complete evaluations, develop treatment plans, and document visits.
- Report outcomes, goals, and projected care frequency.
- Adapt to various home environments, regardless of patient background.
Qualifications & Skills
- Current state licensure in physical therapy.
- Valid driver's license, vehicle insurance, or access to transportation.
- Preferred CPR certification or ability to obtain within 90 days.
