Responsibilities

• Provide therapy services to patients in a SNF setting, assessing and developing treatment plans, delivering therapeutic interventions, and ensuring patient progress.
• Deliver quality patient care, collaborate with healthcare teams, document treatment activities, and contribute to patient recovery goals.

Qualifications

• Valid licensure as a Physical Therapist
• Strong clinical skills
• Ability to work independently
• Excellent communication skills
• Prior experience in a SNF or similar setting preferred
