Responsibilities
- Assess and evaluate patients' functional status, mobility, endurance, and pain to determine care needs
- Develop and implement individualized treatment plans in collaboration with physicians and the healthcare team
- Perform interventions to improve patient well-being, pain relief, and restore or maintain function
- Monitor and document patient responses, progress, and treatment outcomes
- Educate patients, families, and healthcare team members about the care plan and progress
- Complete evaluations, progress notes, and develop plans of care adhering to state guidelines
- Report patient progress, outcomes, and treatment adjustments as needed
Qualifications & Skills
- Current state licensure in Physical Therapy
- CPR certification
- Valid driver's license, vehicle insurance, or access to transportation
- Ability to work effectively in diverse home environments and with clients of all backgrounds
Pay
Compensation ranges from $34.23 to $61.15 per hour, depending on experience and location.
