About the role

Provide physical therapy services in acute, home care, and rehabilitation settings within Yolo County, following physician-approved care plans.

Responsibilities

  • Assess and evaluate patients' functional status, mobility, gait, and range of motion (ROM).
  • Develop and implement individualized treatment plans.
  • Instruct patients and caregivers in the use of therapy devices and exercises.
  • Report patient progress and changes to physicians.
  • Supervise home health aides.
  • Refer patients for additional services as needed.
  • Document patient progress and clinical summaries.
  • Participate in peer consultations and staff competency assessments.
  • Ensure compliance with relevant regulations and licensure requirements.

Requirements & Qualifications

  • Minimum two years of physical therapy experience.
  • Basic Life Support (BLS) certification.
  • Strong communication and organizational skills.
  • Ability to evaluate home environments and instruct patients and families effectively.
  • Must provide supervisor references and meet employment eligibility criteria regarding prior employment with associated facilities.

Mileage is billable according to GSA guidelines with proper documentation. Travel between clinician's home and first visit is not reimbursable. Must adhere to dress code if scrubs are not worn and participate in scheduled meetings.
