Part Time Physical Therapist

Responsibilities

• Evaluate and treat patients following an individualized plan of care.
• Provide excellent care for our patients.
• Provide excellent customer service to our patients and family members.
• Collaborate with facility staff to ensure excellent care for patients.
• Evaluate new patients, develop plans of care and set measurable, functional goals.
• Treat patients, answer their concerns and educate them on their injury, progress and probable outcomes.
• Supervise Physical Therapist Assistants.
• Evaluate patient progress, revise plans of care and reset goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

Qualifications

• Current license to practice as a Physical Therapist in the state of [State]
• Experience working in a skilled nursing facility (SNF) setting preferred.
• Excellent interpersonal and communication skills.
• Ability to work independently and as part of a team.
• Strong problem-solving and critical thinking skills.
