Role Overview
A Home Health Physical Therapist provides personalized care to help patients regain strength, mobility, and independence in their homes.
Key Responsibilities
- Plan, assess, and administer prescribed physical therapy treatments to optimize patients’ physical function.
- Evaluate and document patients' strengths to support physician assessments and treatment planning.
- Develop and implement individualized rehabilitation programs aligned with physician plans and patient goals.
- Provide clear instructions and oversee performance of PTAs and Home Health Aides.
- Maintain thorough documentation of patient observations, interventions, and progress.
- Report patient progress to physicians, care teams, patients, and families, submitting required documentation.
Qualifications & Skills
- Current, unrestricted Physical Therapy license.
- CPR certification.
- Experience collaborating with healthcare professionals.
- Valid driver’s license (patient-facing activities require travel).
- Adherence to TB screening and driver safety programs.
Preferred Experience
- One year of experience as a Physical Therapist.
