Role Overview

A Home Health Physical Therapist provides personalized care to help patients regain strength, mobility, and independence in their homes.

Key Responsibilities

  • Plan, assess, and administer prescribed physical therapy treatments to optimize patients’ physical function.
  • Evaluate and document patients' strengths to support physician assessments and treatment planning.
  • Develop and implement individualized rehabilitation programs aligned with physician plans and patient goals.
  • Provide clear instructions and oversee performance of PTAs and Home Health Aides.
  • Maintain thorough documentation of patient observations, interventions, and progress.
  • Report patient progress to physicians, care teams, patients, and families, submitting required documentation.

Qualifications & Skills

  • Current, unrestricted Physical Therapy license.
  • CPR certification.
  • Experience collaborating with healthcare professionals.
  • Valid driver’s license (patient-facing activities require travel).
  • Adherence to TB screening and driver safety programs.

Preferred Experience

  • One year of experience as a Physical Therapist.
