Part-Time Physical Therapist

About the role

We are seeking a full-time Physical Therapist in Home Health to assess and develop treatment plans for patients, focusing on improving mobility, reducing pain, and maintaining functional ability.

Responsibilities

• Evaluating patients' functional status and therapy needs
• Developing and implementing personalized treatment plans within state guidelines
• Providing interventions to enhance well-being and track progress
• Collaborating with healthcare team, patients, and families to educate and inform about care plans
• Documenting patient visits, progress notes, and treatment outcomes
• Reporting changes in patient condition to supervising staff
• Delivering care in diverse home environments, respecting patient diversity

Requirements

• Current Physical Therapy license in the state of practice
• Valid driver's license, vehicle insurance, or access to transportation
• Preferred CPR certification or ability to obtain within 90 days

Pay

Salary ranges from $97,888 to $146,832 annually, based on experience and qualifications.

Schedule
