About the role
Seeking a licensed Physical Therapist to deliver comprehensive therapy services for senior participants in a person-centered care program.
Responsibilities
- Assess participants' physical needs through in-person evaluations and ongoing assessments.
- Develop and implement individualized treatment plans aligned with recognized standards.
- Administer therapy treatments, measure effectiveness, and adjust plans as necessary.
- Recommend adaptive equipment, home modifications, and ensure safety in participants' environments.
- Educate participants, families, and staff on procedures, exercises, and safety techniques.
- Collaborate with an interdisciplinary team to create and update care plans.
- Document evaluations, treatment progress, and care activities accurately and timely.
- Train staff on body mechanics, equipment use, and mobility techniques to promote safety and independence.
- Participate in team meetings, quality improvement initiatives, and organizational activities.
- Maintain professional certifications, continuing education, and adherence to safety and confidentiality policies.
Qualifications
- Advanced degree in Physical Therapy from an accredited institution.
- Current state license to practice Physical Therapy.
- Preferred experience working with elderly or frail populations, ideally within interdisciplinary teams.
- Valid driver’s license and auto insurance.
