We are committed to bringing critical services to communities across the U.S. to improve access to treatment for over 26,500 patients. Our clinics are all outpatient and office-based, with locations in Georgia, Kentucky, New Jersey, North and South Carolina, Pennsylvania, Tennessee, Texas, and Virginia.

About the role

A day in the life of a Patient Navigator includes:

  • Greeting patients and answering clinic phones.
  • Assisting patients in understanding their treatment options and resources.
  • Serving as an essential link between patients and all other care.
  • Facilitating appointments for consults and support services within established service standards, including social workers, physicians, etc.
  • Coordinating multidisciplinary planning conferences, developing concise patient summaries for use by the care team, and documenting recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
  • Following patients throughout the care continuum, including inpatient admissions, and collaborating with inpatient care management.
  • Working with staff and others to collect data, track outcomes, and support strategic planning processes.
  • Responsible for outreach efforts to establish and maintain positive working relationships with physicians, office staff, nurses, and social services.
  • Assisting the Center in stocking supplies and supporting the Patient Ambassador team.
  • Other duties as assigned.

Requirements

Must hold one of the following credentials/experiences:

  • RN, LPN, or BSW
  • Bachelor’s Degree WITH at least 2 years of lived addiction experience.
  • Associate's degree or CMA WITH 4 years of lived addiction experience.

Schedule

  • Monday: Pennsauken 8:30am-3:00pm
  • Wednesday: Pennsauken 1:30pm-6:00pm
  • Thursday: Lanoka Harbor 2:30pm-7:00pm
  • Friday: Rio Grande 8:30am-12:00pm
