About the role
Join a collaborative, multidisciplinary care team focused on delivering high-quality, personalized care to seniors. In this role, you will work onsite at a senior primary care clinic, partnering with physicians, pharmacists, care coaches, and behavioral health specialists to provide comprehensive wellness visits, chronic condition management, physical exams, and minor injury treatment.
Responsibilities
- Manage adult patients aged 65+ with chronic conditions
- Utilize EMR systems to document and track patient care
- Support whole-person health through coordinated care
Requirements
- Master’s degree as a Nurse Practitioner (NP) or completion of a Physician Assistant (PA) program with board certification
- Active, unrestricted license to practice
- DEA registration
- Experience in geriatric or primary care
Preferred Skills
- Experience in value-based care
- Proficiency in EMR systems
- Ability to obtain necessary licenses and provider numbers
