Responsibilities
- Provide effective, equitable primary care to adult Medicare patients, including routine and acute visits, wellness exams, care coordination, and patient education.
- Practice independently while collaborating with physicians and a multidisciplinary team.
- Lead population health initiatives, assist with phone triage and outreach, conduct home visits if needed, and participate in promotional activities.
- Work closely with operational leadership and support a network of specialists and hospitals.
Qualifications
- Master's or Doctorate in Nursing.
- Active certification in Family, Adult-Gerontology, Adult, or Gerontological Nurse Practitioner.
- Valid state license with DEA registration.
- US work authorization.
- Preferred experience in primary care.
- Understanding of HCC and ICD-10 coding.
- Bilingual skills advantageous.
