Part-Time Nurse Practitioner

About the Role

We are seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care, internal medicine, or geriatrics. This role involves collaborating with a multidisciplinary team to deliver personalized, coordinated healthcare to residents in long-term care settings, primarily within nursing homes or assisted living communities enrolled in Medicare Advantage plans.

Responsibilities

• Assess medical and mental health needs of residents.
• Develop and modify care plans in collaboration with the healthcare team.
• Prescribe medications, order diagnostics, and monitor patient responses.
• Exercise independent judgment while consulting with supervising physicians.
• Ensure compliance with regulatory standards.
• Collaborate with RN case managers and facility staff to improve quality of life and prevent hospitalizations.
• Provide training and supervision to staff as needed.

Requirements

• Active license as an APRN (FNP, AGNP, AHACNP) or PA.
• Degree from an accredited nurse practitioner or physician assistant program.
• At least 3 years of geriatric clinical experience.
• Proficiency with Electronic Health Records (EHR) and Microsoft Office.
