Part Time Nurse LPN/RN

About the Role

The Registered Nurse is responsible for delivering patient care to assigned patients using the nursing processes of assessment, planning, implementation, and evaluation in accordance with federal, state, and local laws, as well as facility standards, policies, procedures, and guidelines.

Responsibilities

• Provides skilled nursing as ordered on the plan of treatment and nursing care plan.
• Ensures each shift or visit reflects skilled nursing care in accordance with physician’s orders.
• Identifies, documents, and follows through on new problems, changes, or resolutions, including:
  • Notifying the Director/Supervisor and physician to obtain necessary orders for intervention per state regulations.
  • Documenting changes on clinical notes, supplemental physical orders, and the nursing care plan.
• Observes aseptic technique as required.
• Performs specific treatments and medication administration as per the care plan.
• Functions as a team member under the direction of the registered nurse.
• Communicates skilled observations and significant changes in patient status to the Director of Clinical Services and/or Clinical Manager.
• Requests modifications or additional orders based on the patient’s clinical status and nursing judgment.
• Documents all physician collaboration in the medical record.
• Demonstrates and documents appropriate utilization of available services.
• Participates in agency-mandated activities, such as in-services, as requested.
• Adheres to MGA policies, procedures, and handbook.
• Completes all clinical documentation accurately and submits it timely per agency policy.
• Represents the company professionally in the community.
• Adheres to the dress code with a clean and neat professional appearance.
• Reports on time and as scheduled to complete work within designated timeframes.

Requirements

• Graduation from an approved certification program, accredited school, or relevant post-graduate education.
• Holds a valid and unencumbered license to practice nursing in the state where services are provided.
• Professional experience as required by state regulations.
• Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater.
• Reliable transportation.
• Current TB results or chest x-ray.
• Current CPR certification as per agency policy.
• Flexibility, organization, and ability to exercise independent and sound judgment.

Physical Requirements

• Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach with hands and arms.
• Vision and hearing capabilities within normal limits, with or without aid.
• Ability to frequently lift and/or move up to 15 pounds and occasionally lift or move 100 pounds or more, if applicable.
• Ability to perform the anticipated level of physical assist required to provide safe client care. If the level of assist exceeds safe capacity, the supervisor must be notified immediately.

Work Environment

• Care is generally provided in private homes, resulting in a highly variable work environment.
• Lighting, acoustics, air quality, temperature, and environmental hazards are unpredictable.
• Occasional exposure to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
• Frequent travel to complete home health visits is required.
