Part Time Master Social Worker

Role: Provides psychosocial services to patients, including those on in-center and home dialysis, using Social Work Theory and practice. Collaborates with the healthcare team to promote patient adjustment, rehabilitation, and quality of life. Responsibilities Assess patients' psychosocial status, strengths, and needs; develop care plans and provide supportive counseling.Educate patients and families about kidney disease, treatment options, and support services; facilitate transplant referrals.Assist with advance directives, DNR discussions, and addressing barriers to treatment adherence.Collaborate on quality improvement activities, monitor adherence, and participate in team meetings.Coordinate with insurance and community resources, and support patient financial and treatment needs.Support staff training, participate in hiring processes, and maintain documentation. Qualifications Master's in Social Work and valid state license.0-2 years' related experience.Knowledge of social work practices, patient education, and community/resources.
