Provides psychosocial services to dialysis patients, working with the healthcare team to promote positive adjustment, rehabilitation, and quality of life.

Responsibilities

• Supports patient education, counseling, care planning, and referral to community resources
• Collaborates on treatment goals and addresses barriers to adherence
• Participates in quality improvement activities
• Assists with patient admissions and insurance coordination
• Provides staff training
• Maintains documentation and adheres to policies

Qualifications

• Master's in Social Work
• Valid state license
• 0–2 years related experience
• Strong communication, assessment, and collaboration skills
