Responsibilities

• Support medication management by assisting with administering injections and preparing oral medications
• Maintain medication records and coordinate with prescribers and pharmacies
• Log medication deliveries
• Conduct weekly health groups
• Participate in team meetings
• Support assessments and documentation

Requirements

• Licensed Practical Nurse (LPN) in New York
• Experience serving mentally ill or chemically addicted populations
• Strong teamwork, communication, and community engagement skills
