Part Time Hospice Social Worker

Iowa City Hospice is growing and seeking a compassionate and dedicated Part-Time Social Worker to join our team. Responsibilities

• Provide emotional support, counseling, and community resources to patients and their families from time of admission until end of services.
• Assess the bereavement needs of the patient’s survivors.
• Conduct comprehensive psychosocial assessments to identify the emotional, social, financial, and support needs of patients and their families.
• Develop and implement individualized care plans.
• Provide counseling, guidance, and emotional support to help patients and families cope with illness and end-of-life challenges.
• Connect individuals with appropriate community resources, support services, and support groups.
• Advocate for patients and families.
• Monitor and evaluate patient progress.
• Maintain accurate and confidential documentation.
• Collaborate with interdisciplinary healthcare teams to ensure comprehensive, patient-centered care.
• Participate in team meetings and ongoing care planning discussions to support quality outcomes and continuity of care.

Requirements

• Bachelor's Degree in Social Work from a CSWE-accredited program required; Master's Degree preferred.
• Current Iowa LMSW, LISW, or LBSW licensure preferred.
• Minimum one year of medical social work, counseling, or related healthcare experience preferred.
• Full-time availability, Monday through Friday, 8:00 AM to 5:00 PM.
• Valid driver's license and current automobile insurance required.

• Strong communication, interpersonal, organizational, and time-management skills.
• Ability to work independently while collaborating effectively as part of an interdisciplinary team.
• Experience conducting psychosocial assessments, care planning, counseling, and patient advocacy.
• Compassionate, empathetic, and patient-centered approach to care.
• Ability to connect patients and families with appropriate community resources and support services.
• Demonstrated professionalism, sound judgment, and commitment to maintaining accurate and confidential documentation.
