Hospice Social Worker Part-TIme

Join our dedicated team! We are seeking a compassionate, skilled, and mission-driven Social Worker (MSW) to join our small and dedicated hospice team. This part-time position offers a unique opportunity to provide meaningful support to patients and families during one of life's most important journeys while working alongside a close-knit interdisciplinary team that values collaboration, respect, and individualized care.

Key Responsibilities

• Conduct psychosocial assessments of patients and families
• Provide emotional support, counseling, bereavement and crisis intervention as needed
• Aid patients and caregivers with advance care planning and end-of-life decision-making
• Connect families with community resources and supportive services
• Facilitate discussions regarding patient goals, family dynamics, coping, and bereavement needs
• Collaborate closely with nurses, physicians, chaplains, aides, and other members of the hospice interdisciplinary team
• Advocate for patient and family needs while promoting dignity, comfort, and quality of life

Qualifications

• Masters Degree in Social Work from a CSWE accredited program
• Licensed as a Social Worker through the Massachusetts Board of Social Worker
• Three years recent (within the past five years) experience in acute, long-term care or home care/hospice, with at least one in hospice or homecare preferred
• Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies
• Strong communication, counseling, and interpersonal skills
• Ability to work independently while remaining an engaged member of a multidisciplinary team
• Compassionate, empathetic, and patient-centered approach to care
