Part-Time Hospice LPN

About the Role

Provide end-of-life hospice care in patients' homes, delivering compassionate, patient-centered support focused on comfort, symptom management, and dignity.

Responsibilities

• Deliver compassionate, patient-centered care focused on comfort and symptom management
• Conduct ongoing patient assessments
• Educate patients and caregivers on end-of-life care and symptom relief
• Ensure compliant electronic medical record documentation
• Work independently while traveling within assigned territory
• Collaborate with an interdisciplinary team
• Provide education and support to patients and families
• Maintain thorough documentation and adhere to compliance standards
• Work autonomously in a home setting

Requirements

• Active LPN license in the state
• Minimum of two years of LPN experience, preferably in hospice care
• Valid driver’s license
• Ability to travel approximately 20% of the time
• Clinical expertise in delivering quality patient care
