About the Role

Provides skilled nursing services to patients in a home health setting, educating patients, families, and healthcare team members according to physician's orders and care plans.

Responsibilities

• Administer nursing care to patients in a home health setting.
• Teach patients and families about health management and care plans.
• Perform services aligned with established care plans and physician's orders.

Qualifications

• Graduate of an approved clinical school.
• Licensed to practice nursing in the state of employment.
• At least one year of nursing experience.
• Basic technology skills required.

Preferred Experience

• Medicare home health or hospice experience.
• Experience as a licensed professional may be reviewed for approval.

Requirements

• Valid state driver license.
